Aspen is open Monday to Thursday from 9:00 AM to 6:00 PM and is closed for lunch from 1:00 PM to 2:00 PM. We remain open on many bank holidays.

Aspen is conveniently located in Raleigh Hills, just 4 miles west of downtown Portland and 3 miles east of Highway 217. The clinic is across the street from Fred Meyer Raleigh Hills Marketplace and Garden Center. TriMet Bus Line 54 offers direct access to Aspen and is accessible from Beaverton TC Max Station via the Red Max Line and Blue Max Line. Bus Routes 56, 61 and 92 also offer service to Aspen. Ample parking is available with disabled person parking and van-accessible space.

How to make an appointment
To schedule an appointment, call (503) 291-7155 and tell us the provider name or the type of service you would like to receive. Same-day appointments are often available.
You may download your registration forms below. Bring your completed paperwork to Aspen. Do not fax or e-mail your forms. Be sure to arrive 15 minutes early for your office visit so you can complete the registration process.
Doctor visits for primary care and chiropractic take approximately one hour for the first office visit. Follow up visits typically take 30 to 40 minutes. Please download and complete the Health Care Registration.
Doctor visits for automobile accident claims take approximately 1 ½ hours for the first office visit. Follow up visits typically take 30 to 60 minutes. Please call your automobile insurance agent to apply for PIP Benefits and be assigned a Claim Number for your medical expenses, then download and complete the Automobile Accident Registration.
Doctor visits for Worker's Compensation claims take approximately 1 ½ hours for the first office visit. Follow up visits typically take 30 to 40 minutes. Please download and complete the following Work Injury Registration.
Acupuncture is scheduled for one full hour. Please download and complete the following Acupuncture Registration. If you were injured in an automobile accident, please bring your claim information with you to your first office visit.
Massage therapy is scheduled for one full hour. You may select from 4 different levels of service, including Swedish Massage ($60), Deep Tissue Massage ($70), Sports Massage ($75) and Rehabilitation Massage ($85). Please download and complete the Massage Registration. Note: If you are a patient of an Aspen doctor, you do not need to fill out these forms to receive massage therapy services.
How to access your medical record
In most cases, a patient has the right to review or to purchase copies of their protected health information (PHI) by requesting access or copies in writing using the Patient Request for Medical Records. Send the completed form by mail or fax to (503) 291-7152. All such requests are handled quickly and efficiently by the provider's Compliance Manager.
Your privacy is important to us
A copy of the Notice of Privacy Practices is given to all patients on their first visit. To protect your privacy, we do not communicate with you regarding health care issues using e-mail because this is not a secure website. You should not send us your personal or health related information over the internet. However, you have the right to request that we communicate about your PHI by alternative means or in alternative locations, including the internet. Your provider will accommodate any reasonable request if it specifies in writing the alternative means or location and provides a satisfactory explanation of how future payments will be handled.
